Question: Intermittent rough engine, loss of power and rapid rpm change

When the engine is cold and warming up it runs and sounds normal. This problem starts when the engine is at operating temperature, and I drive below 40 mph after driving on a highway for 20 minutes. This problem is more frequent or doesn't go away during city driving and when it's hot outside. While this issue is occurring, if I accelerate too much then I hear squealing noises from the engine.

For a while I had an engine code for a small leak in the intake but that code went away. I performed a smoke test and couldn't find any leaks. Then I briefly had an engine code for a bad O2 sensor. I replaced that sensor but was not the problem. Because of the code readings and their strange behavior combined with this issue I thought it would be the ECM. I have replaced the ECM and the problem still persists. I am at a loss at this point. Please help.
